New Patients are asked to complete our New Patient Paper prior to your scheduled appointment time. Please bring the completed paperwork to your appointment. Additionally, new and existing patients may complete a Medical Records Release in order to have your dermatological or other pertinent health records sent to our office.
You are considered a new patient to the practice if you have never been seen by any provider in our practice before, or have not seen by any provider in our practice for a period of more than three years.

Note: Please allow up to 3 business days for medical records requests to be completed. Records will be sent via mail or fax - if you are requesting records to be faxed, please provide a valid fax number. Records 25 pages and longer will not be faxed.
You must bring the following to your appointment with you:
- Completed New Patient Paperwork
- Driver's License (or photo ID)
- A physical copy of your insurance card/information
If you do not provide a physical copy of your insurance card, you will be asked to complete and insurance waiver, and you will be required to bring a physical copy of your insurance card to your next visit. It is your responsibility to ensure that you are providing the correct and most current insurance information to our staff.
If you cannot download and complete the paperwork prior to your appointment, we ask that you arrive at least 15 minutes earlier than your scheduled appointment time in order to complete the paperwork here in the office.
If you are a new patient, please note that you are considered late and need to reschedule your appointment if:
- You have completed paperwork and arrive more than 10 minutes after your appointment time
- You need to complete paperwork and arrive less than 10 minutes prior to your appointment time
If you are a returning patient, we allow a 15-minute grace period after your appointment time. After 15 minutes, you will be considered late and will need to reschedule your appointment.
Patients who fail to cancel scheduled appointments at least 24 hours in advance will be charged a $25 Late Cancellation fee.
Patients who fail to arrive for a scheduled appointment and do not give any notice will be considered a No-Show and will be charged a $25 No-Show fee.
Patients who no-show or give late cancellation notice three times or more will be dismissed from the practice.
Our office hours are from 7:30 AM - 4:00 PM Monday through Thursday and 7:30 AM - 3:00 PM Friday.
